<title>Signs of Morphofunctional Insufficiency of the Fallopian Tube in the Development of Ectopic Pregnancy</title>

Isroilov</p><p><b>The</b> <b>aim</b> of the research was to study morphological data on the identification of morphofunctional insufficiency of structural elements of the fallopian tube wall. <b>Background</b><b>.</b> An ectopic pregnancy (graviditas extrauterina seu ectopica) is a pathological pregnancy in which a fertilized egg develops outside the uterine cavity. The frequency of ectopic pregnancy in relation to the number of gynecological diseases, ranges from 1 to 6%. The causes of ectopic pregnancy are varied. <b>Material</b> <b>and</b> <b>methods</b><b>.</b> 144 patients with a diagnosis of ectopic pregnancy were admitted to the Department of Emergency Gynecology of the Jizzakh branch of the Republican Research Centre of Emergency Medicine in 2021. The majority of women were represented in the age range from 21 to 34 years (68.0%). The following surgical interventions were performed: 91 cases of laparotomies, 53 laparoscopies. <b>Results</b><b>.</b> If normally the proper plate of the folds was represented by a loose, unformed connective tissue, then in our studies, various pathomorphological changes were found in the stroma of the folds: in some cases, inflammatory infiltrates predominated in the composition of the lamina propria, in other cases, sclerotic and dysregenerative changes occupied most of the stroma, in the third cases, foci of hemorrhage and necrosis were found. <b>Conclusion.</b> The development of inflammatory, sclerotic and dysregenerative changes in the unformed connective tissue proper lamina of the mucous membrane of the fallopian tube is accompanied by a violation of the elasticity of structural elements and leads to morphofunctional insufficiency in the form of a violation of the egg cell advancement. The defeat of the muscular layer of the fallopian tube wall by inflammatory, sclerotic and dysregenerative changes leads to a weakening of peristalsis and egg retention in the lumen of the tube, which may be accompanied by implantation to the tube wall.</p>]]></description>

<title>Tactics of Treatment of Patients with Fournier Gangrene in Patients with Diabetes Mellitus</title>

The study was conducted within the purulent surgery and surgical complications of the diabetes department at the multidisciplinary clinic of Tashkent Medical Academy, spanning the timeframe from 2015 to 2022. The successful management of Fournier's gangrene in patients with diabetes mellitus involved intricate surgical interventions that led to prompt scrotal wound healing. The resulting wound defects were addressed through diverse skin plastic and reconstructive procedures utilizing local tissue, thereby averting disability in this challenging patient population.</p>]]></description>

In recent years, this syndrome has attracted close attention from cardiologists, endocrinologists and general practitioners. This is due to the widespread occurrence of this symptom complex in the population. Forty patients with MS were examined. All persons determined by the content IL-6, IL-17 in the blood serum before and after immune disorders correctiveby Ursosan forte. It is established that the MS increases the level of IL-6 and IL-17. The therapy had a positive impact on cytokine status. The activation of cytokine system occurs in metabolic syndrome and manifests as a significant in-crease of IL-6 and IL-17 in blood serum.</p>]]></description>

<title>Identifying the Key Anatomic Structures Contributing to Reduction Loss Following Hybrid Stabilization of Osteoporotic Thoracolumbar Spine Fractures</title>

Hybrid stabilization has been established as a viable treatment strategy for unstable osteoporotic thoracolumbar fractures. However, reduction loss remains a concern, and the specific anatomical structure responsible for this loss is not well-defined.Methods. This retrospective study was conducted at Bukhara Regional Multidisciplinary Medical Center and involved patients aged 61 and older who underwent hybrid stabilization following acute and unstable osteoporotic vertebral body fractures in the thoracolumbar spine. Posterior stabilization was performed using short-segmental and minimally invasive techniques with cement-augmentation of all pedicle screws. The minimum follow-up period was 2 years, and outcome parameters included reduction loss, relative loss of height of adjacent intervertebral discs, fractured vertebral body, and a reference disc (the intervertebral disc superior to the stabilization) between postoperative and latest lateral radiographs. Implant positioning and loosening were also analyzed. Results. The study included 29 patients, mainly female (72%), with a mean age of 73.3 ± 6.0 years. Fractures consisted of 26 incomplete burst fractures and 3 complete burst fractures, primarily located at the thoracolumbar junction (86%) and midlumbar spine. The mean follow-up time was 36 months (range: 24–58 months). The mean reduction loss was 7.7° (range: 1–25), and significant relative loss of heights was observed in both adjacent intervertebral discs, the reference disc, and the central vertebral body. The superior adjacent disc height had the highest loss and was significantly greater than the reference disc. The relative loss of central vertebral body height and reduction loss showed significant correlation. No implant loosening was observed in any patient. Conclusions. Moderate reduction loss was observed 3 years after hybrid stabilization of unstable osteoporotic vertebral fractures of the thoracolumbar spine. Significant loss of heights was observed in both adjacent intervertebral discs and the central vertebral body, with the highest loss occurring in the superior adjacent disc. The superior adjacent intervertebral disc and the central part of the fractured vertebral body were identified as potential contributors to reduction loss.</p>]]></description>

Sh.</p><p>A single distant metastatic lesion is defined as one that has the possibility of local treatment, regardless of the primary lesion of the stomach and regional lymph nodes. There are no large-scale prospective randomized controlled clinical trials to provide evidence-based evidence for the treatment of gastric cancer with relapse or single distant metastasis. Most of the evidence comes from retrospective or small-scale studies. For patients with non-radically resectable primary tumor or PS ≥ 2, the main treatment strategy is to treat recurrent and metastatic gastric cancer or better supportive treatment. For patients with radically resectable primary lesion and regional lymph nodes and PS = 0-1, the main treatment strategy is based on the treatment of recurrent and metastatic gastric cancer, and an optional strategy is individualized decision-making. The optimal therapeutic option for such patients should be discussed in an interdisciplinary commission.</p>]]></description>
